Date of Injury - August 18, 1978, Friday, at approximately 10:30 p.m.

Location of plant or place where accident occurred - US Route 13 and Clifton Park Drive

Describe fully how accident occurred and what employee was doing when injured: Trooper Kelly was in pursuit of a violator when he lost control of his vehicle and struck a telephone pole.

Nature and location of injury: Puncture of front scalp area over forehead.

Resume of violations - Said named officer involved in chase on above date due to lack of good judgement officer was involved in accident.
